Support Coordinator
Hellesdon Hospital, Norwich
Permanent, Full Time (37.5 hpw)
Salary £19,000 to £20,125 pa rising to £23,500 through length of service, pro rata Plus brilliant benefits!

We have a fabulous opportunity for you to join our awesome team. We support our brilliant customers who are admitted to the acute mental health wards across Norfolk, this role is based in Norwich. We support customers whilst they are on the wards in Norfolk by ensuring that their social needs are met in preparation for discharge to take place in a timely fashion to avoid a delay in discharge and free up beds across Norfolk on the acute mental health wards.
 
Areas of support can be housing, benefit or employment. An important part of the role is liaising with other professionals to ensure that everyone involved with the customer understands the customer’s needs. You will also need to attend multi agency meetings and ward reviews updating colleagues on the customer’s progress.

About our team
You’ll be working alongside our fabulous team who are super passionate about supporting customers to reach their goals and celebrating their achievements. To us we’re not just a team we’re a “work family”. Our manager is Tracey Baker who has worked at Home Group for just over 11 years.
 
There are 8 of us in the team and we don’t just love to support our customers, we’re there for each other too! Sarah has been part of the team for 8 years and when she isn’t at work enjoys time with her family, Ady has also been a discharge facilitator for 3 years and with Home Group for over 4 and is passionate about music, Tom has been with Home Group 8 years and a DF for 5 and has a holiday home in Portugal, Eve has also been in the team for over 6 years and is always busy going to music concerts and camping and cooks fab cakes, Gemma has been with the team 3 years and has a daughter who is 2 & ½ years old, so is a full time mum as well. Last year the team expanded and Lucy & Quinn joined us they have masses of enthusiasm and are great role models.
 
Working hours
Your working hours will be Monday – Friday 9-5 although there is flexibility to suit you.

Looking for a more flexible working pattern? You can also join us on a flexible basis by being part of our bank.  You can work hours that fit around your current commitments or even pick up some additional shifts to your main role to earn some extra cash and get paid on a weekly basis.
 
About you
You’re passionate and motivated to help vulnerable people make positive and lasting changes to their lives. You’ll impress us with your ability to work on your own initiative, positively influencing people from all different backgrounds.
 
You’ll help change our customer’s lives (and our services) for the better by undertaking risk assessments, support planning, goal setting and regular reviews. You’ll create bespoke and personalised support plans with and for your customers.  Each plan will look to meet our customer’s individual needs and aspirations, making sure they meet safeguarding and confidentiality requirements. You’ll also have responsibility for coordinating our support workers, apprentices and volunteers, and working collaboratively with external agencies, to assess and engage support for our customers.

To make sure that we keep our records straight, and customers safe, it's really important that you are technologically savvy (or at the very least not a technophobe!) You’ll be given the latest IT kit, which could include a Surface Pro tablet and smart phone, supported by our new online integrated IT system which means that you don’t have to be in the office as you can complete support plans on the go.
 
To make sure our customers are well supported, it would be helpful if you have knowledge in regards to housing, benefits and mental health. Don’t worry, we won’t drop you in the deep end, we’ll help to you understand how things work at Home Group!

As this is not a community based role, you do not need to be able to drive. If you do use your car on occasion we’ll pay you expenses to cover this use!
 
When you join, you’ll need to have a valid Enhanced Check under the Disclosure and Barring Service. The great news is that we will pay for that to be done if you don’t have a transferrable one.
